You must provide specific and verifiable facts that will substantiate your
appeal. Copies of pertinent affidavits (auto repair slips, medical slips, etc.)
may be requested. Separate form required for each violation, unless the
violations are directly related. Appeals must be filed within 10 business
days of ticket issuance. If the appeal process is utilized, the assessment for
the denied appeal shall be the full amount.

If you have any
supporting documentation for your appeal (e.g. pictures, etc.), please send
them via email to email@example.com. Please
make certain to reference your
name, ticket date and ticket number in the subject line of the email.
You may request a personal appeal within 10 business days of denial date.
A second written appeal may be submitted in lieu of personal appeal. You will
receive your decision by e-mail. If you have any questions concerning your
appeal, please contact Campus Parking Services at (815) 753-1045 or by e-mail
at firstname.lastname@example.org. Do not assume your
citation has been waived. The decison of the Parking Appeals Committee is
